feat(flow-capture): FLOW-CAPTURE-LIVE-KN-b gateway proxies + T5 refuse-all

Wave 2: lock personalSelfApply NOT_ADMITTED for all flow_capture kinds; proxy observe/list/propose/dismiss gateway→bridge; PROPOSAL-LIFECYCLE B note. Capture envs stay default off. BV pass (13/13).
