feat(auth): durable MCP OAuth refresh via shared strong store (Phase A)

Wire KnowtationOAuthProvider into createGatewayRefreshStore({ consistency: 'strong' }) with agent meta, scope-aware REST guard for mcp_access, and offline-lock assert. Hermes Hostinger spike was partial → Rank 1↔2 swap (device code primary for Phase B).
