docs: add proper test pyramid — Level 1–4 with TC-0 through TC-full

Stop chasing non-deterministic staging failures. Document the correct test pyramid: hypercorn-direct → nginx-local → staging-direct → staging-CF. Each level must pass 3× before advancing. Level 1 tests not yet written. Next action: test_wire_e2e_hypercorn.py, TC-0.
