fix: created_by's legacy fallback must also check .author, not only .agent_id (musehub#215)

muse branch --json's created_by legacy fallback (used for branches with no creation record, per musehub#216) only ever read the tip commit's agent_id. A human-authored commit (no --agent-id passed, .author set instead) made created_by unconditionally null -- not because nothing is known, but because the one field that IS known was never consulted.

last_committer already handled this correctly (checks agent_id, then falls back to .author). Unified both fields onto one shared tip_authorship computation so created_by's legacy fallback gets the same correct behavior, and the duplicate fallback logic is gone.
