docs: supercharge agent-guide.md Code Intelligence section

- Add layered intel framing (Micro/Meso/Macro altitudes) - Expand reflex table with hotspots, gravity, entangle, find-symbol, detect-refactor, semantic-test-coverage, velocity - Add trigger phrase table mapping agent thoughts to commands - Add task-typed rituals (before modify / refactor / delete / release) - Replace 'Three questions' with 'Eight questions, eight commands' - Update sub-agent boilerplate to mention full command set
