Fix final stale references after unified object store migration

- Remove false step 5 from migrate_commit_ids docstring (never wrote to .muse/commits/) - Rewrite tools/demo.py::build_dag to use get_all_commits() and read_snapshot() instead of reading directly from legacy .muse/commits/ and .muse/snapshots/ dirs
