docs(#192): document muse hooks in agent-guide.md

The hooks feature (musehub#192, shipped v0.2.1rc3) was only documented via --help text and code docstrings -- absent from the canonical command reference every agent in this workspace is pointed at via CLAUDE.md's @include. Adds a full "Hooks — Pre-Commit Gates" section (format, commands, muse commit integration, the dogfood --fail-if-out-of-sync example) plus five entries in the master command-reference table.
