docs: revert migrate hub-scoping/domain-integers rows from agent-guide.md

Reconsidered: this table is @included into every Claude Code session across the whole workspace, forever. muse migrate hub-scoping/domain-integers are one-time bridge tools for a closed, two-person population (gabriel, aaronrene) -- derive_identity_key has required hub as a mandatory kwarg since #221 shipped, so the codebase can never generate a new pre-hub-scoping identity again. Once aaronrene confirms he's migrated, these commands have zero remaining users, ever. That doesn't clear the bar for permanent space in a doc every future session pays context cost for, regardless of relevance -- matches the project's own no-legacy, no-deprecated philosophy, applied to docs rather than code. --help text on both commands already covers discoverability for the rare case someone does need to find them.

The two-person coordination (aaronrene confirming his migration) belongs in the conversation thread that requested it, not in a doc every future agent loads.
