docs: expand cache plan with all seven testing tiers and docstring standard

Each phase now specifies required coverage across: - Unit, Integration, E2E, Stress, Data integrity, Performance, Security

Includes docstring template, test gap backlog for phases 1-3, and full Phase 4 test spec before any production code changes.
