docs: add devdocs-breadcrumb + devdocs-phase-header to phases 04-10

Phases 04-10 were missing the breadcrumb trail and phase badge/title/desc block that phases 00-03 already had. Replaced the old devdocs-title + devdocs-lead (and partial devdocs-phase-label) pattern with the canonical devdocs-breadcrumb + devdocs-phase-header structure in all seven files.
