feat(coupling): Phase 1 — migration 0011, add co_changes + file_a indexes

Adds ix_intel_coupling_repo_co and ix_intel_coupling_repo_file_a to musehub_intel_coupling for efficient leaderboard queries. Updates MusehubIntelCoupling.__table_args__ to match.
