feat(intel): migration 0012 — velocity extended columns

Add prior_modified, prior_active_commits, window_size, commits_analysed to musehub_intel_velocity; add ix_intel_velocity_repo_active index.

Update MusehubIntelVelocity model and VelocityProvider to write all new fields on every run. Enables the velocity detail page and richer current-vs-prior comparison (Phase 3).
