SEC-KN-4a: record operator ratification of freeze D1/D2 (both option A)

Explicit operator selection received and quoted verbatim in freeze section 12.1: - D1 (security): fail closed. No X-User-Id fallback and no truncation for the canister author; a create without X-Actor-Id stores an empty author and apply refuses DELEGATION_AUTHOR_UNVERIFIED. R1.5 stands as written. - D2 (irreversible): one-shot non-identity migration hook, with the identity restore as a required follow-up release (SEC-KN-4c, freeze gate T4) scheduled in the same operator session as the T1 upgrade. R1.4 stands as written.

Section 12.1 preserves the governance distinction round 2 enforced: a general instruction to proceed is not a selection, and an authoring session may never ratify its own escalations. Ratification covers the SEC-KN-4b CODE build only; T1-T4 remain Tier 3 and unexecuted.

A round-3 freeze review runs on the fully amended artifact so clearance rests on a reviewer verdict rather than this session's judgement that round 2's blocker is gone. SEC-KN-4b starts only on pass.
