SEC-KN-4a: freeze review round 2 — revert self-ratification, tighten R2.1/R3 and migration wording

Round-2 independent review confirmed all 8 round-1 amendments hold and found no remaining technical defect in the spec. It blocked on governance: section 12.1 had recorded operator ratification of the escalated D1 (security) and D2 (irreversible) decisions inferred from a general 'continue', which converts a human gate into a default. Reverted to UNRATIFIED; the header now defers to 12.1 as the single status.

Also amended from round-2 findings: - R2.1 and the section 6 integration row said an 'absent or malformed' principal_ref is re-derived and applies, contradicting R3(2)'s refusal for any non-empty mismatch. Narrowed to absent/empty-after-trim/non-string, with an explicit note that the mismatch refusal must never be softened to make a test pass. - Data-integrity idempotency row scoped to agent_identity; the pre-existing consent duplicate-append recorded as residual RR6 (Tier 2 persistence shape, out of 4b). - Repeat-deploy consequence restated as 'fails compatibility or silently resets' — Motoko's behavior here is not provable from this tree, and T4 is required either way.

Governance sync (SD-17): ROADMAP + OVERSEER-HANDOVER in this commit. ok review --freeze = pass. SEC-KN-4b not started; no merge, no deploy.
