docs: inject explicit HTML anchors in code-domain.md section headers

Adds <a id="..."> before each of the 12 numbered section headers so the Contents table links resolve reliably regardless of GitHub's anchor- generation algorithm. Same treatment as plumbing.md / porcelain.md.
